Case One (20 points)

Ms. Beadle was referred to the Boone Speech Center. Her history includes resection of a small tumor from the left perisylvian area. She underwent inpatient rehabilitation during the 5 weeks following surgery. She now wishes to continue her treatment as an outpatient.

The discharge summary from the rehab facility states that "Ms Beadle exhibits a primarily receptive aphasia with significant lexical retrieval deficits. Her self-monitoring is improving but is negatively impacted by mildly disrupted executive functions."

Based on this history, develop an assessment plan. Your plan will be evaluated based on the following criteria:

Case Two

Mr. Benson is 81 years old and was admitted to Happy Acres Long Term Care Facility 3 years ago following a massive right hemisphere CVA leaving him with dense left hemiplegia. He has now been diagnosed with a "mild" left hemisphere CVA and you have received a referral to evaluate him.

1. Based on this information, indicate how you will plan to assess the patient (put a check in the appropriate boxes) (5)

Area Assess carefully Screen Will not assess
Oral Motor
Cognition/Executive function
Receptive language
Expressive language
Speech production
Swallowing

2. Mr. Benson exhibits nonfluent speech characterized by sound errors and articulatory groping. These behaviors are most consistent with what diagnosis? (5)

3. The oral motor exam revealed bilateral facial and lingual weakness, with left sided weakness more pronounced than the right. There is also evidence increased resistance to passive movements of the tongue. What impact would you expect these impairments to have on speech production and swallowing function? (5)

4. Based on the information provided, what type(s) of dysarthria would you expect Mr. Benson to exhibit and why? (10)

5. Develop a "recommendation statement" for Mr. Benson, including whether treatment is indicated, of what nature, frequency, and duration. (15)

Case Three

Mrs. Brown was referred with a 6 month history of ALS. The referral states she has "primary bulbar palsy" with only mild weakness noted in the LUE and LLE.

1. Based on this history, would you expect to observe (X all that apply): (5)

2. Regarding the dysarthria, would you expect Mrs. Brown, at this time, to exhibit (X all that apply) (5) 3. Mrs. Brown exhibits imprecise articulation, hypernasality, breathy voice, and monopitch. Her speech rate is normal although she exhibits short phrases. These behaviors are most consistent with what diagnosis? (5)

4.Based on the assessment findings, develop three treatment goals for Mrs. Brown. The three goals should address impairment, activity, and participation, and all should be objective and measurable. (15)

Impairment goal:
Activity goal:
Participation goal:

4.Develop a 30 minute lesson plan to address these goals. (10)
