3. Assessment case #3. 27 year old female, self-referred, with complaints of slurred speech. She is an elementary teacher who reports that she can speak for several minutes clearly but that her speech begins to break down and becomes steadily worse. It improves if she rests for several minutes.

For each case, provide long term goals and short term objectives. Indicate the target level for each goal or objective (e.g., impairment, disability, handicap). In addition, provide a lesson plan for a 30 minute therapy session, indicating which goals and objectives will be targeted by the specific activities. Each case is worth 20 points.

5. Therapy Case # 1. 35 year old male s/p viral encephalitis with resultant spastic quadriplegia (severe spastic dysarthria as well). Current speech functioning: undifferentiated vowels, maximum sustained phonation 1.5 seconds.
